СФИHКТЕР ОДДИ И ЕГО РОЛЬ В ПАТОГЕHЕЗЕ ЗАБОЛЕВАHИЙ ЖЕЛЧHО-ПАHКРЕАТИЧЕСКОЙ ЗОHЫ

На основании анализа литературы и собственных исследований предлагается гипотеза развития дисфункции сфинктера Одди (ДСО).
ДСО является реакцией сфинктера в ответ на дискинезию двенадцатиперстной кишки (ДПК). Она выражается в периодическом повышении тонуса сегмента ДПК, в который открывается СО. Дискинезия ДПК развивается при избыточном количестве соляной кислоты, при хронических запорах, колите и пр. ДСО является причиной дискинезии желчных путей, образования в них камней и развития хронического панкреатита. ДСО усугубляется во времени. Она начинается с повышения тонуса СО. Постепенно возникает его недостаточность. Как при высоком, так и при низком тонусе СО появляются условия для рефлюкса дуоденального содержимого в протоки. Дисфункция сфинктера Одди может быть частью более общего нарушения моторики желудочно-кишечного тракта, и, следовательно, ориентация только на СО не является решением проблемы. Отсюда вытекает практический вывод: если приходится удалять желчный пузырь, необходимо искать причину ДСО и решать проблему, не дожидаясь появления постхолецистэктомических симптомов.
