 ñòàòüå ïðåäñòàâëåí îáçîð èññëåäîâàíèé, ïîñâÿùåííûõ ïðèìåíåíèþ àóòîòðàíñïëàíòàöèè ïàðàùèòîâèäíûõ æåëåç äëÿ ïðîôèëàêòèêè ïîñëåîïåðàöèîííîãî ãèïîïàðàòèðåîçà. Ðàññìîòðåíû èñòîðè÷åñêèå è êëèíè÷åñêèå àñïåêòû ïðîáëåìû, à òàêæå ïåðñïåêòèâû åå ðåøåíèÿ. Àíàëèç ëèòåðàòóðû ïðîäåìîíñòðèðîâàë, ÷òî íàðÿäó ñ ñîõðàíåíèåì ïàðàùèòîâèäíûõ æåëåç in situ, èìåþùèõ óäîâëåòâîðèòåëüíîå êðîâîñíàáæåíèå, ýôôåêòèâíîé àëüòåðíàòèâîé ïîääåðæàíèÿ ïàðàòèðåîèäíîé ôóíêöèè ÿâëÿåòñÿ ïàðàòèðåîèäíàÿ àóòîòðàíñïëàíòàöèÿ (ÏÀ), ÷òî îñîáåííî àêòóàëüíî â îòíîøåíèè æåëåç ñ ñîìíèòåëüíîé æèçíåñïîñîáíîñòüþ. Íå âûçûâàåò ñîìíåíèÿ íåîáõîäèìîñòü âûïîëíåíèÿ àóòîòðàíñïëàíòàöèè ñëó÷àéíî óäàëåííûõ èëè ïîâðåæäåííûõ ïàðàùèòîâèäíûõ æåëåç.  òî æå âðåìÿ äî íàñòîÿùåãî âðåìåíè íå ðàçðàáîòàí íàèáîëåå îïòèìàëüíûé âàðèàíò ÏÀ, êîòîðûé áû ãàðàíòèðîâàë ñîõðàíåíèå ïàðàòèðåîèäíîé ôóíêöèè. Ñ öåëüþ óëó÷øåíèÿ ðåçóëüòàòîâ ÏÀ íåîáõîäèìî ïðîâåäåíèå äàëüíåéøèõ èññëåäîâàíèé, íàïðàâëåííûõ íà ðàçðàáîòêó íîâûõ õèðóðãè÷åñêèõ ïîäõîäîâ è ìåòîäîâ îöåíêè æèçíåñïîñîáíîñòè ïàðàùèòîâèäíûõ æåëåç. Äëÿ áîëåå ãëóáîêîãî ïîíèìàíèÿ ìîëåêóëÿðíûõ îñíîâ ÏÀ òðåáóåòñÿ èçó÷åíèå îñîáåííîñòåé ðåâàñêóëÿðèçàöèè, ðåèííåðâàöèè è ôóíêöèîíèðîâàíèÿ ðåèìïëàíòèðîâàííîé ïàðàòèðåîèäíîé òêàíè.
