ВЕHОЗHЫЕ ТРОМБОЗЫ В ТРАВМАТОЛОГИИ И ОРТОПЕДИИ: ТРУДHОСТИ ДИАГHОСТИКИ

В обзоре приводится сведения об актуальности проблемы нарушения гемостаза после операций при переломах длинных трубчатых костей и больших ортопедических хирургических вмешательствах, относящихся к операциям с высоким риском развития тромбоэмболических осложнений, а также своевременной и полноценной диагностики этих грозных осложнений. Ценность данных клинического обследования у этих пациентов невелика. Инструментальные методы позволяют диагностировать уже сформировавшиеся тромботические массы. Этого недостаточно для проведения соответствующих профилактических и лечебных мероприятий. Предтромботическое состояние удаётся выявить благодаря использованию лабораторных методов. Из лабораторных тестов, доступных на сегодняшний день, только определение уровня D-димеров могут помочь объективизировать степень тромботического риска у пациента. Однако этот тест обладает высокой диагностической чувствительностью только в поздней посттравматической фазе и вместе с тем обладает низкой специфичностью, особенно у пациентов травматолого-ортопедического профиля. Поэтому определение D-димеров может быть недостаточным для исключения диагноза венозного тромбоза в этой группе пациентов.
