АСПЕКТЫ АHТИКОАГУЛЯHТHОЙ И ДЕЗАГРЕГАHТHОЙ ТЕРАПИИ У ПАЦИЕHТОВ ПОСЛЕ ПЕРИФЕРИЧЕСКИХ АРТЕРИАЛЬHЫХ РЕКОHСТРУКЦИЙ В ИСТОРИИ И HА СОВРЕМЕHHОМ ЭТАПЕ

Применение антикоагулянтов и дезагрегантов у пациентов с периферическим атеросклерозом многие годы является неотъемлемой частью терапии. Роль и их сочетание претерпевали многочисленные изменения с развитием представления о механизмах действия и их влиянии на периферическую гемодинамику. Кроме того, применение этих препаратов играет решающую роль, когда речь идет о пациентах с критической ишемией нижних конечностей, перенесших реконструкцию периферического артериального русла. Зачастую именно грамотно подобранная таблетированная терапия в послеоперационном периоде может стать тем самым «ключевым звеном» в спасении конечности. Не стоит забывать, что атеросклероз – процесс мультифокальный, поражает также и коронарный, церебральный бассейны. Патологические процессы в этих системах могут значимо отразиться на здоровье и качестве жизни, зачастую носят инвалидизирующий характер, нередко угрожая жизни пациента. В настоящее время противоречивость клинических и практических сведений об эффективности различных схем антикоагулянтной и антиагрегантной терапии не вызывает сомнений. К примеру, не поддерживается достоверными исследованиями необходимая длительность таблетированной терапии. Большое число устаревших исследований, их недостаточная «масштабность» являются камнем преткновения для определения современного единого стандарта периоперационной терапии пациентов с периферическим атеросклерозом. Выявление «идеальной» схемы позволит не только улучшить ближайшие и отдаленные результаты оперативных вмешательств, но и выполнять эффективную профилактику грозных осложнений со стороны сердечно-сосудистой системы в целом.
