ХИРУРГИЧЕСКОЕ ЛЕЧЕHИЕ ПЕРЕЛОМОВ ЗАДHЕЙ СТЕHКИ И ЗАДHЕЙ КОЛОHHЫ ВЕРТЛУЖHОЙ ВПАДИHЫ

Цель. Оценить эффективность применения хирургического лечения при смещённых переломах задней колонны и задней стенки вертлужной впадины.
Материал и методы. Проанализирован опыт хирургического лечения 31 пациента с переломами задней стенки и колонны вертлужной впадины. Показаниями к выполнению операций явились переломы со смещением более 2–3 мм, интерпозиция осколков в суставе, не вправленные переломовывихи в сроки до 3–4 недель с момента травмы. Во всех случаях использовался задний доступ Кохера-Лангенбека. Фиксация переломов осуществлялась с помощью реконструктивных пластин.
Результаты. Клинические результаты лечения оценены в сроки до 7 лет после операции. Отличные результаты получены у 48%, хорошие и удовлетворительные у 29%, неудовлетворительные у 23% пациентов. Рентгенологический исход лечения соответствовал характеристике клинического результата.
Заключение. Точная анатомическая репозиция и стабильная фиксация с помощью пластин позволяет достичь оптимальных результатов лечения данных повреждений и в ранние сроки начать реабилитационные мероприятия.
