СПИHАЛЬHАЯ АHЕСТЕЗИЯ И ГИПОТЕHЗИЯ: МЕХАHИЗМЫ, ФАКТОРЫ РИСКА, МЕРЫ ПРОФИЛАКТИКИ И КОРРЕКЦИИ

Гипотензия является наиболее распространенным побочным эффектом спинальной анестезии (СА). Частота СА-индуцированной гипотензии в общей популяции составляет 15-33%, у пациентов старших возрастных групп достигает 80%. В молодом и среднем возрасте основной детерминантой СА-индуцированной гипотензии является снижение постнагрузки и венозного возврата, обусловленное преимущественно депонированием крови в емкостных сосудах нижних конечностей. В акушерской практике существенную роль играет аорто-кавальная компрессия, сердечный выброс при этом не изменяется. У пациентов пожилого возраста основной предпосылкой развития гипотензии на фоне СА является повышенный базальный тонус симпатической системы. У пациентов данной категории, помимо, снижения общего периферического сосудистого сопротивления (ОПСС), играет роль и снижение сердечного выброса. Факторами риска гипотензии являются следующие: уровень сенсорного блока выше Th6, гипертоническая болезнь, алкоголизм, экстренный характер хирургического вмешательства. С целью профилактики СА-индуцированной гипотензии целесообразно снижение доз местных анестетиков. Данные большинства исследований не подтверждают эффективность расширенной инфузионной терапии в отношении профилактики гипотензии и ее коррекции. Основную роль в коррекции гипотензии играют вазопрессоры – фенилэфрин в акушерской практике, эфедрин или дофамин у пациентов пожилого возраста.
