ВЛИЯHИЕ ТАКТИКИ ИHТЕHСИВHОЙ ТЕРАПИИ HАРУШЕHИЙ ГЕМОСТАЗА HА ИHТРАОПЕРАЦИОHHУЮ ПОТРЕБHОСТЬ В ТРАHСФУЗИИ КОМПОHЕHТОВ КРОВИ И ТЕЧЕHИЕ ПОСЛЕОПЕРАЦИОHHОГО ПЕРИОДА ПРИ ТРАHСПЛАHТАЦИИ ПЕЧЕHИ

Цель. Сравнить эффективность интенсивной терапии нарушений гемостаза, основанной на данных тромбоэластометрии, с эффективностью интенсивной терапии нарушений гемостаза по результатам стандартных коагуляционных тестов.
Материал и методы. В исследование включены 70 пациентов, которым выполнялась трансплантация печени. Пациенты были рандомизированы на две группы. В исследуемую группу вошли 35 пациентов, которым интенсивная терапия нарушений гемостаза проводилась на основании анализа данных тромбоэластометрии, в контрольную группу – 35 пациентов, у которых коррекция нарушений гемостаза основывалась на результатах стандартных коагуляционных тестов.
Результаты. Проведение интенсивной терапии геморрагического синдрома, основанной на данных тромбоэластометрии, позволило уменьшить объем кровопотери с 1800 (1300-2000) мл до 1200 (1000-1700) мл и потребность в трансфузии эритроцитной массы (ЭМ) и свежезамороженной плазмы (СЗП) с 1447 (920-2120) мл и с 2600 (2000-3400) мл до 1056 (692-1286) мл и 500 (0-1200) мл соответственно. Уменьшение количества трансфузий компонентов крови привело к снижению частоты развития острого почечного повреждения (с 88,6% до 65,7%) и сокращению продолжительности лечения в условиях отделения реанимации (с 6 до 5 суток) и в стационаре (с 18 до 16 суток).
Заключение. Проведенное исследование подтвердило высокую эффективность интенсивной терапии нарушений гемостаза, основанной на данных тромбоэластометрии, по сравнению с интенсивной терапией нарушений гемостаза по результатам стандартных коагуляционных тестов.
